This license is required for the use of amusement devices such as pinball machines, pool tables, arcade games, and jukeboxes in commercial establishments.
How to Apply
You can apply online via the self-service portal. You can provide all required information through the application process.
The licensing period for this license is July 1 – June 30. Renewal applications must be submitted through the self-service portal.

Fees
The annual amusement device license fee is $50 per device. Establishments with 5 or more amusement devices must also apply for an annual entertainment license. The annual entertainment license fee is $200.
